1st Lt Henry C. Lewis Jr., pilot of the B-24 Liberator 42-50453, took off from station 124 in Tibenham, Norfolk, UK on a bombing mission over Karlsruhe. They collided with the B-24 42-95291 of 2nd Lt Robert G. Phillips at 1257. He was listed as MIA until 1988.
