James Willard Cook was born on December 11, 1936 in Welch, West Virginia. He was one of nine killed when their Bell UH-1H Iroquois helicopter crashed on the edge of a field near Unteraltertheim while flying from the unit's barracks at Baumholder to the Army training grounds at Grafenw�hr. CSM Cook is now buried in the Beaver Creek Cemetery, Grand Rapids, Wood County, Ohio, USA.
