Horace Baker Forman III was born on March 4, 1894, in Maryland. He served in the 3rd Aviation Instruction Center as a Second Lieutenant during World War I. 2Lt Forman was killed in an airplane accident in Issoudun, France, on September 14, 1918. His head was hit by the propeller of his plane as he was getting out of the airplane. He is now buried in the St. Mihiel American Cemetery, Thiaucourt, France. He also has a cenotaph in the Dale Cemetery in Ossining, Bew York.
